A Real Example

One older patient entered a Creda program after experiencing repeated falls. During the first assessment, the nurse learned that he was taking a medication differently from the way it had been prescribed. The nurse explained the instructions, alerted the medical practice and checked back with the patient and his caregiver. The conversation connected what the doctor intended with what was actually happening at home.

Will Creda cost me anything?

It depends on the program and your insurance coverage. Some Creda programs are covered by your health plan or another organization involved in your care. Others may include a cost to you.

If there is a known cost, we’ll explain it before you enroll so there are no surprises.

How is my health information protected?

We take the privacy and security of your health information seriously. Creda uses secure systems and follows applicable privacy requirements when collecting, using, and sharing your information.

What should I do in an emergency?

Creda is not an emergency service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Your program materials will also explain how to reach your doctor or clinical care team for time-sensitive concerns.

Will Creda Replace my Doctor?

No. Creda works alongside your healthcare team. Your doctor remains responsible for your medical care and treatment decisions. Creda provides additional support between appointments and helps keep your healthcare team informed when appropriate.

How often will Creda contact me?

It depends on your program and your needs. Your Creda care team may check in regularly by phone, text, app, or another approved method. We’ll explain what to expect when you get started

Can my caregiver or family member be involved?

Yes, when permitted by your program and with your permission.

A caregiver or family member can help with appointments, goals, questions, and next steps so the people supporting you can stay connected.
